Starting from the Beginning:
Care often begins before discharge from the hospital or care center. If the patient is under a doctor's care, we consult with that physician before our Home Care begins. To obtain an understanding of your personal needs, we send a Registered Nurse or Home Care Supervisor to conduct an on-site assesment at no cost to you. Upon assesment, we develop a personal home health care plan that meets your needs and budget.
